Membership Renewal Process

Please submit the following by June 30, 2011. Just click on RENEW MY MEMBERSHIP NOW! to complete the process online.

1. Pay the balance due for 2011 membership renewal.

2. Complete the short Annual Member Questionnaire to update your information for our records by clicking RENEW MY MEMBERSHIP NOW! or download a hard copy of the survey and send it to us.

3. Send us a copy of your Most recent IRS Form 990 – OR – Most recent audited financial statement.

4. Send us a copy of your Most recent grant docket/List of grants made.

5. Send us a copy of your Most recent annual report.
